Paperwork Forms

You may have additional paperwork upon arrival depending on your diagnosis and insurance provider.

 

Please bring your insurance card and a photo ID with you.

 

To further expedite the process, call with or email your insurance information so that we may verify our insurance prior to arrival.

Secondary to being the only place in St. Louis that does 60 minutes of 1 on 1 care and having a long waiting list, it is important that our clients give us a 24-hour cancellation notice.

If you need a PCP referral please have your MD send it over prior to your first appointment.

If you need authorization please let us know, so we can get it prior to your first appointment.  Not getting authorization could lead to higher payments on your end.
